Event Description
A customer reported the unit was locking when stepping on the foot pedal during a vitrectomy procedure.The event did not prevent the conclusion of the procedure.There was no harm to the patient.Additional information has been requested.
 
Manufacturer Narrative
No further information was able to be obtained from this customer.With no additional, related information provided, the customers reported event was not able to be confirmed.A review of the customer¿s complaint history for the last 24 months did not show any previous complaints of this kind against the system.The system was manufactured on november 20, 2010.Based on qa assessment, the product met specifications at the time of release.The footswitch was manufactured on october 18, 2010.Based on qa assessment, the product met specifications at the time of release.Footswitch was received for evaluation.An assessment of the returned sample revealed the left and right heel switches would stick when pressed.The left heel switch was opened up and dried balanced saline solution (bss) ingress was seen on the pin and pin hole.The right heel switch was unable to be opened due to a stuck screw.Evidence of bss ingress indicates the same cause as the left heel switch.There was no information on the system.The root cause of the reported event can be attributed to bss ingress on the pins of the heel switches, causing them to stick.An internal investigation has been opened to address the bss issue.The manufacturer internal reference number is: (b)(4).

The system was examined and the reported event was replicated.The footswitch was replaced to resolve the issue.The system was tested and found to meet product specifications.A review of the customer¿s complaint history for the last 24 months did not show any previous complaints of this kind against the system.The system was manufactured on november 20, 2010.Based on qa assessment, the product met specifications at the time of release.The footswitch was manufactured on october 18, 2010.Based on qa assessment, the product met specifications at the time of release.The system was found to meet specifications.The manufacturer internal reference number is: (b)(4).
