A service visit was performed.Investigation including root cause analysis is in progress.A supplemental mdr will be filed a necessary in accordance with 21 cfr 803.56 when additional reportable information becomes available.Additional information has been requested.(b)(4).
 
Event Description
A nurse reported a system message was displayed and the system froze during a combined cataract and vitrectomy surgery.The system message appeared during the vitrectomy stage.There was no device in patient's eye at the moment.They tried to reset the system unsuccessfully, the nurse clicked 'skip priming' and the system message reappeared.They tried to restart the system again and the system froze.A 'hard reset' was needed and then the surgery could be completed.No patient harm confirmed.The reporter confirmed the event did not take place during fluid air exchange.Additional information has been requested.
 
Manufacturer Narrative
Evaluation summary.The system was examined.The company representative did not indicate finding any issues that would be associated with the reported event.The supervisor module printed circuit board (pcb) was replaced.The system was tested and found to meet product specifications.The product met specifications at the time of release.The root cause cannot be determined conclusively.(b)(4).
