(b)(4).A review of the manufacturing records for the device verified that the lot met all pre-release specifications.The device was not returned; consequently, a direct product analysis was not possible.
 
Event Description
On (b)(6) 2016, a gore acuseal vascular graft was implanted in an arteriovenous access application.The implant was successful.On (b)(6) 2016, the graft was cannulated for dialysis access.Homeostasis was confirmed after holding pressure for 15 minutes.It was reported to gore that the patient presented with a swollen arm immediately.A hematoma was observed near the cannulation site.On the same day, the graft was explanted and a central catheter was placed.It was reported that the patient did not have sufficient subcutaneous tissues and the puncture was accidentally made on the side of the graft which made the homeostasis difficult.
