The reported lot number could not be matched to the reported device.Therefore, the lot expiration and device manufacture dates are unknown at this time.However, the complainant stated that the device was used prior to the expiration date.(b)(4) peg tube difficulty placing/retracting.The device has been received for analysis.Upon completion of the failure analysis of the complaint device, if there is any further relevant information from that review, a supplemental medwatch will be filed.
 
Event Description
It was reported to boston scientific corporation that an endovive safety peg kit pull method was used during a percutaneous endoscopic gastrostomy placement procedure performed on (b)(6) 2016.According to the complainant, during the procedure, they had difficulty pulling the peg tube through the stomach.Reportedly, the incision size is 1cm.The procedure was completed with this endovive safety peg kit pull method.There were no patient complications reported as a result of this event.The patient's condition at the conclusion of the procedure was reported to be fine.
 
Manufacturer Narrative
A visual examination of the returned blue pull wire revealed no visible residue and a physical examination of the wire found no issues.No other components were returned.It was noted that the condition of the returned device could not be verified because the complaint device was not returned.The customer only returned the blue pull wire.Based on all gathered information the investigation fails to determine a definite root cause.A review of the device history record (dhr) lot number 19115837 was performed; no anomalies were noted.A search of the complaint database confirmed that no similar complaints exist for the specified batch.
