Event Description
A programming anomaly was identified while performing a periodic review of the in-house programming history database.On (b)(6) 2012, the device was interrogated two time and then system diagnostics was performed, showing the device was working appropriately.A final interrogation was performed confirming the vns therapy parameters were set to the intended settings.The next recorded visit showed the patient's device had been programmed off and the settings were indicative of a faulted diagnostic test.The faulted diagnostic test occurred at some point between the visit on (b)(6) 2012 and the initial interrogation on (b)(6) 2015.
