(b)(4).The fsr returned on 19-aug-2016, the batteries failed so the fsr will return to install new batteries.The fsr arrived on 23-aug-2016 to install new batteries, battery back-up is now functioning and the pm inspection was completed successfully.The unit operated to manufacturer specifications and was returned to clinical use.No parts will be return to the manufacturer for evaluation as the fsr disposed of the batteries.

Event Description

The field service representative (fsr) reported that during preventive maintenance (pm) of the device, the battery back-up failed when system-1 (aps1) was disconnected from wall outlet.The aps1 has not been connected to alternating current (a/c) since (b)(6) 2016 (the last time that it was used clinically).The fsr is charging overnight to see if the batteries will charge.There was no patient involvement.

The complaint is confirmed by the field service representative (fsr).The fsr advised and made the customer aware that they need to charge the unit at least once every month.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
