It was reported by (b)(6) that the chuck with key device had general wear and tear, the lot and reference were not properly visible, and the motor does not couple properly.During service and evaluation, it was observed that the adaptor had general wear and tear, there was poor legibility of the reference and lot, and the motor connection pin was found damaged which causes oscillation during operation.It was not reported if this event occurred during a surgical procedure.It was reported that there was no delay in a procedure due to the event.It was not reported if a spare device was available for use.There was no patient involvement.There were no reports of injuries, medical intervention or prolonged hospitalization.The exact date of this event was unknown.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.
