Event Description
It was reported during a cesarean section, the nurse inserted a bakri balloon in the vagina and added 200ml of fluid.Approximately 2 hours later the nurse proceeded to fill another 50ml into the balloon through the syringe and noted fluid leaking from the vaginal canal.The nurse contacted the regional manager and deflated the balloon to remove it from the patient.The regional manager confirmed the top of the balloon had burst.After the bakri device was removed, the physician examined the patient and confirmed no further procedures or intervention was required.The nurse confirmed the patient was reportedly "doing well post procedure.".

Investigation - evaluation: the cook bakri postpartum balloon with rapid instillation components was not returned/received for an evaluation.The customer report of the top of the balloon burst could only be confirmed based on the customer¿s testimony.Based on the information available a definitive root cause could not be established.A review of production and quality documentation did not identify any specific issues with current manufacturing or quality controls that may have contributed to this incident.A review of the device history record was not able to be performed as the lot number of the device was not provided.Per the quality engineering risk assessment; no further action is required.Cook medical has notified the appropriate personnel and the will continue to monitor this device via the complaints database for similar complaints.
