Event Description
|
A patient was being treated for post partum haemorrhage.A bakri balloon was selected by an obstetrician who is very familiar with the product and its application.An attempt was made to fit the stopcock with the blue luer tap, but it was found the connection could not be made securely with the device.After a small volume of saline was delivered through the stopcock the connection would blow off and the saline sprayed.Approximately 5 attempts were made to deliver the saline through the blue tap over 20 minutes, but the problem persisted.The product was discarded, then a new bakri balloon was opened and used without incident.

Investigation - evaluation: a review of the specifications and quality control of the complaint device was conducted during the investigation.The complaint device was not returned therefore, no physical examinations could be performed; however, a document based investigation evaluation was performed.There is no evidence to suggest the product was not made to specifications.The stopcock on this device was inspected to ensure that there was no damage or defect that would impair proper functionality.Based on the information provided, no product returned and the results of our investigation, a definitive root cause could not be determined.We will continue to monitor for similar complaints.Per the quality engineering risk assessment no further action is required.
