It was reported to boston scientific corporation on november 28, 2016 that a wallflex enteral colonic stent was to be used in the colon during a stent placement procedure performed on (b)(6) 2016.According to the complainant, the stent was placed to treat 3cm malignant stricture.Reportedly, the patient¿s anatomy was not tortuous and had not been dilated prior to stent placement.According to the complainant, during the procedure, the physician was able to deploy the stent.However, during removal of the delivery system, the tip of the delivery system got caught on one of the cells of the stent and pulled the deployed stent completely out of the patient.The procedure was not completed because another wallflex enteral colonic stent was not available.There were no patient complications reported as a result of this event.The patient's condition at the conclusion of the procedure was reported to be fine.
