The zoll autopulse platform in complaint was returned to zoll on 12/27/2016 for investigation.However, investigation is still in progress.A supplemental report will be filed when investigation has been completed.

It was reported that during service check, the autopulse platform (sn: (b)(4)) was displaying error message "system error, out of service, revert to manual cpr".No patient involvement was reported.

|
The customer reported complaint of the autopulse platform was displaying error message "system error -out of service revert to manual cpr" when powered on was confirmed during archive review and initial functional testing.The root cause of the reported complaint was due to the drive train motor brake assembly air gap was too wide, out of specification.The drive train motor replacement would remedy the complaint.Archive data review indicated error message user advisory (ua) 139 (unable to hold compression position) on (b)(6) 2016.The platform failed the initial functional testing due to the error message "system error- out of service revert to manual cpr" displayed when the unit was powered on.Historical complaints were reviewed for service information related to the reported complaint and there was no previous history of related complaints for autopulse sn (b)(4).
