It was reported to boston scientific corporation that a solyx sis system was used during a mid urethral sling placement procedure performed on (b)(6) 2017.According to the complainant, during implantation on the right side, instead of implanting the device in the obturator internus muscle, the device was implanted into the trigone of the bladder.This perforation of the patient's bladder and was identified through cystoscopy.To remove the mesh, he attached a scissor to the cystoscope, cut the right-side anchor out of the mesh, then pulled it out with the scope through the urethra.The remaining parts of the sling were removed vaginally.The left anchor remained in the left obturator internus, but the mesh was cut close to the anchor such that 95% of the mesh was removed.The procedure was completed with another of the same device.A foley catheter was placed to be left in for a week, and the patient will return for a follow up check up in a week.
