Event Description
It was reported via phone call that the customer's blood glucose was approximately 400 mg/dl due to a bent infusion set cannula.The customer stated that he also had frequent issues with the site getting pulled off due to his work environment.Troubleshooting was declined.The insulin pump was not returned for analysis.
