((b)(4).Reported event of wire broken.Investigation results: visual evaluation of the returned rx cytology brush found that the handle was slightly kinked.The device was in opened or extended position upon received.The bristled portion of the brush was present, properly formed and without issue.Functional analysis showed that the device failed to retract and the handle was disassembled finding that the pull wire had been kinked and broken at the hypotube joint.The reported complaint was confirmed, the pull wire broke.The review and analysis of all available information failed to indicate a root cause and is therefore, undeterminable.A review of the device history record (dhr) was performed and no deviations were found.A search of the complaint database revealed that no other complaints exist for the specified lot.

It was reported to boston scientific corporation that an rx cytology brush was used during a procedure.According to the complainant, during the procedure, the brush broke inside the catheter.They also noticed that the device failed to extend and retract.There were no known patient complications reported as a result of this event.Attempts to obtain additional information regarding this event have been unsuccessful to date.Should additional relevant details become available, a supplemental report will be submitted.
