(b)(4).Evaluation is in progress, but not yet concluded.The manufacturer's subsidiary evaluated the unit.The unit powered on and booted properly.Visual inspection of a printed circuit board assembly found a burnt capacitor.The unit was returned to the manufacturer for further evaluation.
 
Event Description
It was reported that during use of the device for a cardiopulmonary bypass (cpb) procedure, the cdi 500 gave off a white smoke about one hour into the procedure.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.Per clinical review: during cpb, smoke was observed to have come from the inside of the cdi 500 monitor.The user immediately powered down the monitor and they unplugged it from the alternating current (ac) power source.The monitor was removed from the operating room and replaced with a back-up.The case was completed successfully, without delay and without associated blood loss.There was no harm observed.
 
Manufacturer Narrative
The reported complaint was confirmed.During laboratory analysis, the product surveillance technician (pst) observed component c26 on the auxiliary printed circuit board assembly (pcba) failed (burned) and was found to be the cause of the smoke.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
