Event Description
The reporter indicated that the surgeon implanted a 13.2mm vticmo13.2 implantable collamer lens, -16.0/1.5/74 diopter, in the patient's right eye (od) on (b)(6) 2016.In the same surgery, the lens was explanted due to lens tear/beak during injection into the eye.On (b)(6) 2016 the lens was exchanged for the same size and diopter lens.The problem was resolved.The customer stated after loading the lens, it was not put in bss.The customer though that is the reason causing the lens tore, and the surgeon has been trained again.As of the date of mdr submission, the lens has not been returned.
 
Manufacturer Narrative
Device evaluation: the product was returned dry, in a micro centrifuge vial with residue on product.Visual inspection found the lens cut in two pieces, missing a piece of haptic and residue on the lens surface.(b)(4).
