(b)(4).The field service representative (fsr) verified the ccma only went to a white screen when powered on.He also observed the smell of electronic smoke.The monitor will be returned to the manufacturer for further evaluation.
 
Event Description
It was reported that during set-up of the device for a cardiopulmonary bypass procedure the central control monitor a (ccma) only came up to a white screen that was blinking on and off.Also the perfusionist noticed burnt smell.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.
 
Manufacturer Narrative
The reported complaint was confirmed.During laboratory analysis, the product surveillance technician observed the central control monitor (ccm) screen to blink white to black.It was determined that the system board computer was the cause of the problem.The smell of burnt electronics was not present and inspection of all internal components revealed no burnt or damaged components.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
