Event Description
It was reported that the bd insulin syringe with the bd ultra-fine¿ needle was difficult to operate.The shield cap was difficult to remove, and the plunger was hard to move.Found before use.No serious injury or medical intervention noted.
 
Manufacturer Narrative
Bd had not received samples or photos from the customer facility for evaluation; therefore, the investigation was limited.A review of the manufacturing records was completed for the incident lot and no issues were identified.Conclusion - without a sample, an absolute root cause for this incident cannot be determined as bd was not able to duplicate or confirm the customer¿s indicated failure mode.
