Event Description
It was reported that this patient was having a generator replacement due to eos.During surgery the surgeon noticed that the lead was loose and not working due to observed high impedance.Therefore the physician also replaced the lead and the surgery became a full system replacement.The patient was said to have had a double mastectomy recently and the physician thought this may have been the reason for the issue.During the left mastectomy it was reported that the general surgeon had to move the generator during surgery.The device has not been received by the manufacturer to date.No other relevant information has been received to date.

It was reported that the product was discarded in pathology at the hospital facility by mistake.No other relevant information has been received to date.
