Event Description
The reporter indicated that the surgeon attempted to implant a 12.6 mm vticmo12.6, -12.50/2.5/117 (sphere/cylinder/axis), implantable collamer lens in the patient's left eye (os) on (b)(6) 2017.The lens was "half implanted" when the plunger overrode the lens and got stuck in the cartridge.When the lens was removed for reloading, a tear in the peripherical haptic was noted.The lens was not reimplanted.On (b)(6) 2017, a similar lens was implanted and it was successful.

Additional data: device evaluation: the lens was returned in liquid inside a lens vial.Visual inspection found no visible damage to the lens.Claim# (b)(4).
