Event Description
The reporter indicated the surgeon inserted a 13.2mm vicm5_13.2 implantable collamer lens, -08.00 diopter, and the lens went into the eye inside out.The lens was damaged while being removed within the same surgery and there was no report of any injury.The lens was exchanged and the problem was resolved.
 
Manufacturer Narrative
Device evaluation - the product was returned dry, in a micro centrifuge vial.Visual observation found the lens missing a piece of haptic and residue on lens.Claim# (b)(4).
