Event Description
|
It was reported that during a cryo ablation procedure, after pulling back the balloon catheter, the mapping catheter snapped.The lasso eventually detached, and made its way through the mitral valve.It was noted that the lasso was able to be snared out of the descending aorta without incident.The case was completed with radiofrequency ablation.No patient complications have been reported as a result of this event.

Product event summary: the device and data files were returned and analyzed.Data files showed that 5 applications were performed with a balloon catheter no issues on the date of the event.Visual inspection of the mapping catheter showed that the pebax was torn, the loop was detached and ribbed beside the electrodes.All the 8 electrodes were available on the ring.The size of the detached part was about 3 inches from the tip and there was a kink around 8 inches from the lemo connector.However, it may have occurred during post procedures.Performance testing was not completed because the device was defective.In conclusion, the reported issues of a broken tip and detachment were not confirmed through data analysis but confirmed through device testing.The catheter failed the returned product inspection test due to a broken electrode-wires loop.If information is provided in the future, a supplemental report will be issued.
