Event Description
A report was received that the patient had loss stimulation.X-ray revealed that the lead had moved to the epidural space.It was noted that two contacts were off of the paddle.The patient underwent a lead replacement procedure.It was also confirmed that all dislodged contacts were removed.The patient was doing well postoperatively.
