The results of the investigation are inconclusive since the reported device was not returned for analysis.Based on the information received, the cause of the reported event could not be conclusively determined.(b)(4).
 
Event Description
During a coronary atherectomy procedure using a csi orbital atherectomy device (oad), patient complications occurred.The target lesion was a complete occlusion and was located in the left anterior descending artery (lad).The lesion was treated with two passes of the oad on low speed.Following the second pass, the pressure of the patient decreased and tachycardia was noted.The device was removed and imaging was performed, and no issue was noted.The patient status continued to decline and entered ventricular fibrillation.Cardiopulmonary resuscitation was performed and the patient was defibrillated for 12 minutes, and a ventricular assist device was inserted.The condition of the patient stabilized and additional atherectomy treatment was performed with the oad.The procedure was completed with balloon angioplasty and stent placement and the patient was in stable condition following the procedure.It was reported that the patient later expired.
