Event Description
The reporter indicated that the surgeon loaded a 13.2mm micl13.2, -5.0 diopter, implantable collamer lens wrong into the cartridge.The reporter stated that it was a loading issue and that cause of event is unknown.There was no patient contact with the device and the backup lens was used.
