Event Description
The reporter indicated that a 12.6mm, vticmo12.6, -09.00/1.5/107 (sphere/cylinder/axis), implantable collamer lens was implanted into the patients left eye (os) on (b)(6) 2018.It was reported that the inside paitent label had an incorrect serial number, the lens was implanted, the axis is aligned according to the label on the box not the inside label.The patient out come is good.
