Event Description
|
It was reported that during a procedure, the power cord of light source 500xl burned during the operation.The place in which the fire started was at a part of a multi plug of power strip.The customer exchanged the power cord and re-connected with another power source, the device was successfully booted.The procedure was completed without patient injuries and complications.

The reported device was not returned to the designated complaint unit for independent evaluation, thus visual inspection and functional testing could not be performed.A relationship, if any, between the subject device and the reported event could not be determined.If the product associated with this event is returned at a future date, this evaluation will be reopened for investigation.

The device was received for evaluation.There was a relationship found between the returned device and the reported incident.A visual inspection was performed on the product and no issue was observed.Complaint of burned power cord was confirmed.The burnt power cord was not found shorted out and still functions.Light source passed functional testing.All functions perform as expected and light output was steady and continuous.The complaint was confirmed and the root cause was determined to be a defective power strip or an improper installation of the power cord into a multi plugged power strip causing an overloaded circuit.A review of the device history records showed there were no indications to suggest that the product did not meet manufacturing specification or would not be able to perform as intended.
