Event Description
Information was received from a consumer regarding a patient who was implanted with a neurostimulator (ins) for dystonia, movement disorders.It was reported that the patient charges propped up in their bed with one hand holding the antenna.The patient didn't think that they moved at all and it would go from 8 bars to 4 to 2.The patient stated that the ins was working very well.The patient then did troubleshooting with patient services and turned the antenna dial through all 4 positions.The patient tried to keep the antenna in place and the importance of antenna placement was reviewed.The results of troubleshooting were unknown.The patient mentioned this was the case since implant.There were no further complications reported.

Medtronic, inc.(medtronic) is submitting this report to comply with 21 c.F.R.Part 803, the medical device reporting regulation.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information in the time allotted and has provided as much information as is available to the company as of the submission date this report.This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic or its employees caused or contributed to the event described in the report.In particular, this report does not constitute an admission by anyone that the product described in this report has any "defects" or has "malfunctioned".These words are included in the fda 3500a form and are fixed items for selection created by the fda, to categorize the type of event solely for the purpose of reporting pursuant to part 803.Medtronic objects to the use of these words and others like it because of the lack of definition and the connotations implied by these terms.This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report.If information is provided in the future, a supplemental report will be issued.
 
Event Description
Additional information received from the consumer reported that even though they tried to put the antenna in exactly the same place, in the same orientation, every time, it seemed they could have all eight coupling boxes lit, or any number fewer.According to the consumer this happened whenever they placed the antenna directly on their skin when they were sitting and not moving.The consumer was asked what was done to resolve the poor coupling and stated nothing was done to resolve the poor coupling as the person they spoke with agreed it could be ¿finicky¿ and difficult to achieve coupling.No patient symptoms or further complications were anticipated.

Event Description
Additional information was received: it was reported that the cause of the poor coupling was not determined.It seemed that the patient had difficulty getting the antenna to couple without several repositioning attempts.The actions taken to resolve the issue were repositioning the antenna as needed.When asked about resolution of the issue the hcp stated it persisted, but the patient was able to eventually achieve coupling with repositioning the antenna.There were no further complications reported.
