Event Description
|
According to the reporter, during laparoscopic ventral hernia repair procedure, device fired a few tacks normally and then reload could not be loaded on the distal tip of the device.The light blue unlock/lock switch was jammed.It was noted also that a little silver ball fell out of distal tip onto mayo stand.They then used another device to resolve the issue in order to complete the case.There was no patient injury.

Evaluation summary: post market vigilance (pmv) led an evaluation of one device.A visual inspection of the returned product noted the distal sleeve was missing from the tip of the shaft.No single use loading units (sulus) were received.The articulation knob functioned properly.A test sulu was unable to be loaded onto the instrument.The device was disassembled for visual evaluation, there was no significant wear noted on the components that would indicate damage to the device.Records from each manufacturing lot are thoroughly reviewed to ensure that products are released meeting all quality release specifications at the time of manufacture.The investigation concluded there were no abnormalities that would have caused or contributed to the reported condition.If information is provided in the future, a supplemental report will be issued.
