A sample product was not returned for analysis.Complaint history and product history records were reviewed and documentation indicated the product met release criteria.Root cause has not been identified.There are six other complaints in the lot.Additional information was requested.The manufacturer internal reference number is: (b)(4).
 
Event Description
A technician reported that during an intraocular lens (iol) implant procedure, the tip of the cartridge split as the lens was injecting into the patient's eye and caused the lens to be partially stuck in the end of the injector.The surgeon was able to pull the lens back out of the eye still stuck in the injector, but it did cause the patient a corneal abrasion.The iol was also damaged.Another lens was implanted instead.The doctor then placed a bandage contact lens on the eye.In a follow up the reporter indicated that the bandage lens was removed at the one week postoperative appointment and the abrasion was healed.
