Event Description
Ppf alleges dislocation, metal wear, metallosis and elevated metal ions.After review of medical records, patient was revised to addressed failure left total hip with instability and metal on metal bearing causing metal on metal debris.Revision notes indicated black fluid, took the head and liner out, took the screw that was there out and copious irrigation was performed.Doi: (b)(6) 2008; dor: (b)(6) 2018; left hip.
