Event Description
It was reported by the sales rep via phone that during a labrum repair procedure, the patient's bone hole was prepared and drilled and while malleting the anchor into the bone, the lupine loop rapide anchor with orthocord slipped out of the patient's bone while removing the drill guide.The sales rep stated that the loop on the anchor was exposed and therefore it was unusable.The anchor was removed from the patient by pulling on the sutures with no debris left behind.The case was completed with another anchor in a new bone hole.There was a two minute surgical delay to remove the anchor.The anchor was discarded by the customer.
