Event Description
|
It was reported to boston scientific corporation that a solyx blue sis system was implanted during a stress urinary incontinence (sui) procedure performed on (b)(6) 2019.According to the complainant, during the procedure, the mesh carrier did not release from the shaft tip of the delivery device and the plastic tube around the shaft was bent.Reportedly, the device worked after third deployment attempt.The procedure was completed with this device.There were no patient complications as a result of the event.The patient's condition at the conclusion of the procedure was reported to be good with no injuries and with no re-operation scheduled.
