Event Description
|
A customer reported that the fs6 and fs8 gas were not recognized during surgery.The surgeon manually performed infusion of the gas.The case was completed without patient harm.A company representative found a crack in the port and replaced the port and the connectors.

|
The company service representative examined the system and replicated the reported event.The coaxial port was cracked.The coaxial cpc connector was replaced to resolve the issue.The system was then tested and met all product specifications.The system manufacturing device history record (dhr) was reviewed.Based on qa assessment, the product met specifications at the time of release.The root cause of the reported event can be attributed to a cracked coaxial port.The manufacturer internal reference number is: (b)(4).
