Event Description
Litigation alleges patient was revised to address pain and discomfort.In addition to what were previously alleged, ppf alleges elevated metal ions, pseudotumor, heart attack, stroke, metal wear and metallosis.Added all other implant due to these allegations.Doi: (b)(6) 2007 - dor: (b)(5) 2014, (left hip).

Event Description
After review of medical records, the patient was revised for failed tha on the left hip, secondary to metal on metal reactive bone and soft tissue disease.Operative notes reported that there was also significant amount of fluid reaction involved which was grey and cloudy.The entire trochanteric region had been destroyed by psuedotumor and lost the attachment of the abductor and bone erosion of the trochanter.There was no external rotator left.Doi: (b)(6) 2007; dor: (b)(6) 2014; left hip.
