Date rec'd by mfr: 21nov2019.The part was returned to the manufacturer for failure investigation (fi).This failure was caused by a defective flash ic u1 (lot code: p33bf70).Installing a good failure investigation (fi) test flash ic u1 corrected the problem with this central processing unit (cpu) printed circuit board assembly (pcba).Submission of a report does not constitute an admission that medical personnel, user facility, importer, distributor, manufacturer, or product caused or contributed to the event.

G4: 04sep2019; b4: 05sep2019.The manufacturer's service technician confirmed the cpu issue.The technician replaced the cpu pcba to address this issue.The unit was checked overall, run in tested, cleaned and functionally tested and no abnormality was confirmed.Submission of a report does not constitute an admission that medical personnel, user facility, importer, distributor, manufacturer, or product caused or contributed to the event.
