There was no patient involvement.The heater-cooler 16-02-80 is not distributed in the usa, but it is similar to heater-cooler 16-02-85, which is distributed in the usa (510(k) number: k052601).Livanova (b)(4) manufactures the heater-cooler system 3t.The incident occurred in (b)(6).A review of the dhr did not identify any deviations or non-conformities relevant to the reported issue.If any additional information pertinent to the reported event is received, it will be provided in a supplemental report.

Livanova (b)(4) received a report that the main plug of a heater-cooler system 3t could not be removed from the socket because the plug plastic had melted and glued to the wall socket.An electrician could remove the plug by force eventually.There was no patient involvement.

H.10: as stated in the previous report, the part was not made available for further investigation.However, the investigation carried out on similar cases from the same hospital is most likely applicable also to this specific case.During the investigation performed by the manufacturer it was confirmed that one pin of the mains connector was exposed to large heat.The heat effect took place with high probability from the outside over the pin to the inside.From an electrical point of view, the plug and the cable were in good conditions, thus, no damage except for the fused point on the plug.The cause of the damage was very likely a faulty hospital wall outlet.
