There was no patient involvement.Livanova (b)(4) manufactures the heater-cooler system 3t.The incident occurred in (b)(6).The livanova field service representative in charge inspected the device and found that the patient pump 2 was blocked.He replaced the pump and the issue was solved.Subsequent functional verification testing was completed without further issues and the unit was returned to service.Through follow-up communication livanova (b)(4) learned that the user did not use the circuit 2 since august 2019 neither procedure nor disinfection procedures as prescribed in the instruction for use and this likely affected negatively the pump leading to the reported event.Indeed, a picture of the pumo has been provided and visible signs of corrosion have been identified.A review of the dhr did not identify any deviations or non-conformities relevant to the reported issue.If any additional information pertinent to the reported event is received, it will be provided in a supplemental report.
