Event Description
The reporter indicated the surgeon implanted a 13.7mm vticm5_13.7 implantable collamer lens, -13.0/+0.5/081 (sphere/cylinder/axis), in the patient's left eye (os) on (b)(6) 2019.The lens was explanted on (b)(6) 2019 due to low vaulting.An iris-fixated lens from a different manufacturer was then implanted.

H3 - device evaluation: lens was returned in a case/vial, dry.Visual inspection found the lens haptic broken.Dimensional inspection found the lens to be within specifications.Claim#: (b)(4).
