The scope cyf-vha serial # (b)(4) was returned for evaluation and repair.Visual inspection was performed and found that the a rubber glue of the scope was chipped and peeling with the threads exposed.The insertion tube was determined to have scratches and a hole on the insertion tube boot.The light guide case tube has leaking from the cut on the boot and the video connector has a cracked m-case.Based on device evaluation, the likely cause of the damages found on the scope was due to mishandling.As a preventive measure, the instruction manual states: do not strike, hit, or drop the endoscope¿s distal end, insertion tube, bending section, control section, universal cord, video connector, or light guide connector.Also, do not bend, pull, or twist the endoscope¿s distal end, insertion tube, bending section, control section, universal cord, video connector, or light guide connector with excessive force.The endoscope may be damaged and could cause patient injury, burns, bleeding, and/or perforations.It could also cause parts of the endoscope to fall off inside the patient.
 
Event Description
It was reported that the scope used during a urological exam had small hole noted on the boot.The customer stated that due to the hole, and the potential to retain a bioburden they would like the scope evaluated.There was no patient harm or injury reported due to the event and the intended procedure was completed.
