Description of event according to short form complaint filed: it is alleged that "[pt] received a cook celect filter on (b)(6) 2014.It is alleged that the [pt] was injured without further explanation.Hospital and medical records have been requested but not yet provided.".
 
Event Description
Patient allegedly received an implant in (b)(6)2014 via the right common femoral vein due to post deep vein thrombosis (dvt).The patient alleges tilt and vena cava perforation.The patient further alleges physical pain, limited activity, and dvt.(b)(6) 2018, per a report from computed tomography; ¿some of the filter struts have penetrated through the wall of the ivc into the pericaval/mesenteric fat.The filter is tilted to the right with its proximal cone lying on the wall of the ivc possibly embedded in it.¿ (b)(6), per a report from ct; ¿the infrarenal ivc filter is in place, slightly tilted to the right.The legs of the ivc filter appeared to traverse the wall of the ivc.There is otherwise no obvious fracture.Compared to the radiograph dated (b)(6)2018, this is unchanged in alignment.(b)(6) 2019, per a report from xray; ¿an ivc filter is again seen at l2 - 3 level.¿.

Additional information has been provided.The patient alleges embedment and a successful percutaneous filter retrieval on (b)(6) 2020.11jun2020, per a report from retrieval report (successful); ¿we then delivered the sheath over the ivc filter to collapse the struts and successfully retracted the filter to within the sheath.We then removed the entire system as a unit while visualizing that the ivc filter remained contained within the sheath.¿.
