The following were reviewed as part of this investigation: patient severity, complaint and lot history, applicable previous investigation(s), labeling, applicable manufacturing records, and applicable fmea documents.Based on a review of this information, the following was concluded: the complaint of a kinked guidewire was confirmed and appears to be related to the use of the device. Two guidewires and one 14 fr dilator were returned for investigation. One of the two guidewire was returned frayed and the other was intact. Both guidewire were observed to contain multiple bends and kinks. The frayed guidewire was observed to have kinks approximately 30 cm from the distal end and a slight bend 5 cm from the distal end.The distal weld tip was present. The intact guidewire was also observed to have kinks approximately 40 cm from the distal end.Microscopic observation of the frayed guidewire revealed the core wire to have an uneven break. The outer diameters of the guidewires were measured and were found to be within specification. One section of the dilator orifice was flared outwards and showed material whitening. The dilator was likely damaged during forceful removal of a kinked guidewire.Based on the condition of the returned samples, possible contributing factors include advancement or retraction of the guidewire against resistance and damage to guidewire during use.A lot history review (lhr) of redr1642 showed one other similar product complaint(s) from this lot number. The complaints for this lot number (redr1642) have been reported from the same facility in (b)(6).
