Event Description
|
A surgeon reported that during a surgical procedure the system froze.The staff tried to reboot however the device was unable to turn off.The power cord was unplugged to turn the power off.The device was then rebooted and the surgery was completed.There was a one and a half hour delay but no patient harm was reported.Additional information has been requested.

The company service representative examined the system and was able to confirm and replicate the reported event.The nonconforming host module was replaced.The power module was replaced as a preventative measure (pm).The system was then tested and met all product specifications.The system manufacturing device history record (dhr) was reviewed.Based on qa assessment, the product met specifications at the time of release.The root cause of the reported event can be attributed to a nonconforming host module.The manufacturer internal reference number is: (b)(4).
