Additional information received from a manufacturing representative (rep) indicated the devices were prepared per the instructions for use (ifu), and it was clarified that "wire access was lost" meant the pipeline failure to open was observed after implanting an detaching the device.Multiple attempts were made to retrieve the device using snares and attempting access from the other side, but nothing worked. The pipeline had not been placed in a vessel bend, there was no resistance or friction more than expected, and no damage observed to the pushwire or catheter. The patient experienced entercranial bleeding prior to the death.
