Event or Problem Description
It was reported that central sterile has discovered some broken items they want replaced.These items were not involved in a surgery.No one was injured by the broken items.Surgery was not delayed.Sales rep is returning all 3 items.Size 7 artic surface piece is has a missing broken piece that i do not have all the parts to.Size 6 artic surface has a bent prong where spring is.Tibial impactor has crack on back that extends to inside impactor.

Product complaint # (b)(4).Investigation summary : examination of the returned device confirmed the reported damage.The noted damage is consistent with material overload through the use of excessive force and the investigation did not establish a need for corrective action.Depuy synthes considers the investigation closed at this time.Should additional information be received, the information will be reviewed and the investigation may be re-opened as necessary.
