Description of event according to short form complaint filed: it is alleged that "[pt] received a cook günther tulip filter on (b)(6) 2017.It is alleged that the [pt] was injured without further explanation.Hospital and medical records have been requested but not yet provided.".
 
Event Description
Patient allegedly received an implant via the right common femoral vein due to deep vein thrombosis and heavy vaginal bleeding.Patient is alleging migration, vena cava perforation, fracture, inability to retrieve and spine perforation.Patient further alleges required medication, fear, weakness, shortness of breath, depression and anxiety.Retrieval report (attempted): ".Presence of small tip clot and clot superior to filter seen on most recent ct's dated (b)(6) 2019 and (b)(6) 2019." "multiple attempts to over sheath and remove the filter were attempted with the 14 french vascular and was unsuccessful." "the pre-removal cavogram shows a gunther ivc filter with fractured posterior thrut." " the filter was abled to be engaged and collapsed however due to significant scarring/ingrowing into the caval wall, filter was not able to be removed safely." or a filter leg being caught in a side branch (e.G., a renal vein).Other potential causes of filter fracture may include excessive force or manipulations near an implanted filter (e.G., a surgical or endovascular procedure in the vicinity of a filter).Retrieval of a fractured filter or filter fragments (including embolized fragments) using endovascular techniques has been reported.Potential adverse events that may occur include, but are not limited to, the following: filter fracture, filter or filter fragment embolization, trauma to adjacent structures.Filter interacts with ivc wall, e.G.Penetration/perforation/embedment.This may be either symptomatic or asymptomatic.Potential causes may include improper deployment; Event Description
Per ct report, "there is a centrally positioned removable filter in the infrarenal inferior vena cava, there is a 2,7 x 1.4 cm bulbous appearing filling defect superior to the tip of the filter hook, which may represent a thrombus," "otherwise there is no filling defect within the inferior vena cava or the visualized lilac veins to suggest deep venous thrombosis." "impression: removable filter appears to be centrally located in the inferior vena cava.The posterior long thrut [sic] appears to have penetrated the posterior wall of the inferior vena cava (which is not uncommon) and has incorporated into or caused paravertebral calcification in the anterior space of l2-3 lumbar intervertebral level".Per ct report, "1.9 x 1.2 cm clot is noted approximately 1cm above the hook of the removable ivc filter at the level of the renal veins.Additionally, minimal clot is seen at the apex of the filter." per ct report, "the ivc filter appears to have a collapsed appearance with the prongs no longer in a fully open within the ivc.The position of the ivc filter also appears to have slightly migrated more superiorly when compared to previous exam, and is noted to be just at the level of the left renal vein, slightly tilted towards the left.There is no findings of underlying prongs extending beyond the lumen of the ivc.Previously noted thrombus extending above the level of the ivc there is no longer seen".

"one of the legs protrudes through the wall of the ivc by 20.0 mm." "the second leg protrudes through the wall of the ivc by 4.4 mm.".
