Block b3 (date of event): the exact date of the event is unknown. The provided event date (b)(6) 2020 was chosen as a best estimate based on the date that the manufacturer became aware of the event.Block h6 (device codes): the problem code 1069 captures the reportable event of cutting wire broken.Block h10: the returned jagtome rx was analyzed, and a visual evaluation noted that the cutting wire was detached and blackened; also the cut of the wire was not smooth that shows it was not a mechanical cut.Additionally, the working length was twisted and torn from the end of the c-channel to distal tip.No other issues with the device were noted. The reported event was confirmed. The failure found could be caused by a peak of voltage or if the device was not in contact with the tissue during energization.Additionally the working length was twisted and torn from the end of the rapid exchange channel to distal tip that it is possible that the factors encountered during the procedure the technique used by the physician during extraction of the device, patient anatomy or the interaction with the scope; could have contributed with the reported event.Based on all gathered information, the most probable cause of this complaint is adverse event related to procedure since the adverse event occurred during the procedure and the device had no influence on event.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.A labeling review was performed and, from the information available, this device was used per the directions for use (dfu) / product label.
