Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date. This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.H3, h6: part: 393.105, lot: 3666416 , manufacturing site: hägendorf, release to warehouse date: 05 april 2011.A manufacturing record evaluation was performed for the finished device lot number, and no non-conformances were identified.Visual inspection: the small universal chuck with t-handle (part # 393.105/ lot # 3666416) was received at us customer quality (cq). The device was received in the closed position with one of the teeth protruding further than the others. The chuck rotates freely, and the rest of the tooth can be seen coming out and in with the chuck rotation. No defects were identified with the loose tooth. No other defects were identified.Functional test: a functional assessment was performed on the device, and one of the tooth is jammed in the assembly. There was no evidence of device breakage on the tooth, it appears the tooth has slid off the inner ring which holds it in place. No physical device breakage was identified but the overall complaint was confirmed as the tooth was jammed in the chuck. Can the complaint be replicated with the returned device(s)? yes; tooth is jammed.Dimensional inspection: dimensional inspection was not possible as device could not be disassembled to access relevant components.Document/specification review: drawing(s) reviewed: current & manufactured revisions. Conclusion: the overall complaint was confirmed for the received small universal chuck with t-handle as the tooth is protruded from the assembly and jammed.Although no definitive root-cause can be determined its possible the device experienced unintended forces during use. There was no indication that a design or manufacturing issue contributed to the complaint. No design issues were observed during the document/specification review.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post market safety surveillance activities.Device was used for treatment, not diagnosis. If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
