Additional information and corrected data: received stated that the initial surgery date is (b)(6) 2019 and the date that the patient returned to have the iol implanted was on (b)(6) 2019.Iol that was implanted was not a johnson and johnson iol.As a result the following fields have been updated: section b3: date of event: (b)(6) 2019.Section d10: device available for evaluation: yes.Section d10: returned to manufacturer on: 6/10/2020.Section h3: device returned to manufacturer: yes.Device evaluation: the complaint lens was received in its original lens case.Additionally, the original folding carton, patient id card, implant notification card, patient stickers, dfu, and an opened inner pouch were received.Visual inspection under magnification revealed what appeared to be dried blood and viscoelastic residue on the optic body and haptics, and that the lens was received cut (but not separated) and a detached haptic (not received), which is consistent with a lens that was handled for removal and replacement.Based on the returned condition of the lens, no further product evaluation could be performed. The complaint issue could not be confirmed, and no product deficiency could be identified.Manufacturing records review: the manufacturing records for the product was reviewed. The product was manufactured and released according to specification.A search revealed that no additional complaints have been received for this production order. Conclusion: as a result of the investigation, there is no indication of a product quality deficiency. All pertinent information available to johnson & johnson surgical vision, inc.Has been submitted.
