Description of event according to short form complaint filed: it is alleged that "[pt] received a cook celect filter on (b)(6) 2011.It is alleged that the [pt] was injured without further explanation.Hospital and medical records have been requested but not yet provided.".

Event Description
Patient allegedly received an implant via the right common femoral vein due to deep venous thrombosis and pulmonary embolism.Patient is alleging inability to retrieve, thrombus, deep venous thrombosis, thrombectomy and balloon angioplasty.Patient further alleges pain, complications, femoral vein catheterization, thrombolytic therapy, pulmonary embolism.Report from computerized tomography (ct): "infrarenal ivc filter in place, with likely acute thrombus within the ivc, iliac veins, and femoral veins." report from venogram: "chronic and acute right femoral vein dvt, which ws treated with balloon angioplasty plus mechanical thrombectomy plus thrombolytic therapy using tpa infustion.The patient also had a thrombus in the ivc filter with significant thrombus burden, which ws treated with mechanincal thrombectomy using agiojet mechanical thrombectomy device.
